We currently have an opportunity for a Social Worker with long-term care experience to join our team in a leadership role.
DUTIES &
Responsibilities:
Plan, develop, organize, implement, evaluate and direct the social service program of the facility Develop and implement policies and procedures for identifying the medically-related social and emotional needs of patients/residents Assist in providing solutions to social and practical environmental problems, including seeking financial assistance, discharge planning (including collaboration with community agencies) and providing referrals to other community agencies when specialized assistance is required REQUIREMENTS:
Current Social Work license in good standing with the state as required Minimum of 1 year of experience in a human services field in a hospital, skilled nursing facility or other related medical facility Ability to effectively organize and prioritize many tasks Skilled communicator, director and motivator
