Family Nurse Practitioner - Value Based Care Opportunity
We are seeking a highly motivated and dedicated Family Nurse Practitioner to join our team.
You will be supported by anentire care team, including a health coach, to deliver care that is oftenoutsourced to specialists in today's broken system.
As a committed physician, you will be focusedon outcomes-based medicine and data-driven population health, and will be eagerto live your passion to serve others and restore primary care to what it wasalways meant to be.
Compensation:
$125,000.
00 - $135,000.

Essential Functions:
o Provide evidence-based primary care to 10 to 15 patients per day,focusing on prevention, delivering practical treatment for chronic and acutecare needs, and providing disease and case management services o Partner with the patient andyour care team, including an onsite Whole Health Coach, to develop effectivebiopsychosocial patient care plans that meet the needs of each unique patient,respecting their diverse backgrounds o Ensure comprehensive care inalignment with the medical home model, including collection of comprehensivehealth history, performing appropriate physical examinations, conductingdiagnostic testing, and prescribing medications - all with the convenience ofonsite lab and pharmacy o Assess patient population needsand work with team members to drive participation in annual whole healthevaluations, health coaching, and other health promotion activities o Provide clinical leadership,including participation in team meetings, clear proactive communication,regular prioritization of key efforts, process improvement, and activelysupporting change management o Rotate with other providers in the region toensure coverage after-hours o Participate in organizationalactivities and committees (e.
g.
weekly provider meetings, Clinical AdvisoryCommittee meetings, Quality Improvement Committee, etc.
) to optimize processesand programs
